Information for patients of Rehabilitation Services.

Prescriptions

You are responsible for obtaining the therapy order from your doctor. The need for a prescription is mostly dictated by your insurance plan.

Insurance Information

Please review your medical insurance plan to ensure it covers therapy.  As a service to our patients, we will contact your insurance prior to your initial visit and verify your eligibility and therapy benefits.  We will gladly share the therapy benefit information gathered but this does not guarantee payment.  Some insurance companies limit services and /or require prior authorization.  If you need assistance, please contact either our office or your primary care provider.

Co-payments

Co-payments are made at the time of your appointment. Our billing department can also assist you with billing plans as needed.

Cancellation and No-Show Policy

Call us in advance if you know that you won’t be able to make your appointment, preferably a 24 hour notice. If you have three cumulative short-notice cancellations and/or no-shows, we will discharge you from our care. You will need to obtain a new therapy order from your physician if you need to resume therapy.

Fragrance

Minimize or avoid the use of fragrance for the health and comfort of all who use the facility, including staff, patients and visitors.

Timeliness

We make every effort to start your appointment on time.  Occasionally, we are delayed by an unexpected event with another patient.  If you arrive 15 minutes late for your appointment, you may be asked to reschedule or the therapist may choose to proceed with the treatment, but your appointment will end at its scheduled time.  For appointment durations of 15 minutes or 30 minutes, we asked that you come in on time.

All patients under the age of 15, or adult patients who require supervision should be accompanied by a parent, caregiver, or other designated adult.

For Medicare patients only

To be in compliance with Medicare, you must see your referring doctor every 90 days while receiving therapy. Talk to your therapist to find out how long they anticipate you will be in therapy. If it is expected that you will see us for longer than two months, we recommend that you make your doctor’s appointment now to prevent a lapse in your therapy treatment. If you need to continue therapy but have not seen your doctor, by law we cannot provide treatment until you do. When you visit your doctor, please obtain a new prescription to continue therapy services. If we do not receive a prescription to continue, we will have to discharge you.
