When it comes to cancer, time is of the essence. A patient’s first visit to the Tuality/OHSU Cancer Center usually comes shortly after a referral from a surgeon, specialist, chemotherapy office or primary care physician. Our team connects with patients as soon as possible to arrange a consultation with one of our physicians.
What to Bring
We advise patients to arrive a few minutes ahead of their appointment to complete all the necessary paperwork. Patients can download and complete our Medical History and Records Release forms prior to their meeting to save time. Our team can also mail these forms upon request. Other documents to bring include:
- List of current medications
- Medical insurance information
- Personal identification (driver’s license, state I.D., or an envelope showing current address)
After a patient’s first visit to the Tuality/OHSU Cancer Center, their oncologist communicates with their primary care physician and referring physician who are also involved in their care. If a patient needs treatment with radiation therapy, our team obtains consent and one of our therapists works directly with patients to schedule next steps, which include simulation and treatment planning. All patient records are kept in our electronic medical record system for the entire course of their treatment.
We know a cancer diagnosis creates many questions and concerns. That’s why the Tuality/OHSU Cancer Center offers a patient advocacy and navigation program that facilitates open communication among patients, their family members, and the appropriate hospital staff. Our patient advocate helps patients understand their rights, hospital policies, services provided, financial responsibility and more.
