Release of Information

Rights to Receive Records

  • A patient, or his/her legal representative, may inspect and/or obtain a copy, or have copies of medical records sent to another facility. 
  • Tuality Healthcare requires a completed and signed written request and authorization for release of health information form before releasing any documents to anyone, including the patient.

Pertinent Information

  • We ask that you specify what components of your medical records you wish to obtain.  Often, the discharge summary, operative report, history and physical contain relevant information to suit your needs.

Frequently Asked Questions 

Will you mail the copies or will I have to pick them up?
We can mail the copies to the address you specify on the Authorization form, or you can make arrangements to pick them up during our hours of operation.

Will you fax medical information?
We will only fax medical records to another healthcare provider or facility for continuing medical care.
